Conditional Formatting Formula not Working

I am trying to create a conditional formatting formula between two columns. In one column I am trying to have all numbers above 10 highlighted, and in other the column all numbers over 120 highlighted. This is the formula I am using: =IF(AND($T3>=10,$V3>=120),TRUE,FALSE)

It is being weird and highlighting cells that don't fit the criteria and ignoring some cells that do (It is also not working when I plug in new column numbers?) I'm not sure what is wrong, I thought my formula was correct?

Also random side note, Is there a way to able this formula to multiple columns that have the same name without having to make a formula for each column number (ex.$A3)and instead the name of the column in the formula? Any help is appreciated! Thanks!
