Can a tool like Little Snitch can detect all spy software trying to establish outgoing coections?

I've been using Little Snitch for a while now, and I've prevented more than a few suspicious outgoing coections, but I am still wondering if a really good hacker could write a program which circumvents Little Snitch.

Would that be possible - if yes, how?
