Search and include some text in Microsoft Word

I don't know if it is possible to do. I have a Word document with a lot of text (sorted and unsorted lists, styles, images, etc) and I want to write specific text depends on the content.

For example, if I have a title and a subtitle I'd like to add a text between them. (Check the images below)

Modified text

This Word file will be converted taking into account these tags so I need to automatice (or try it) this part.

Is it possible to do that?
