Block VPNs accessing my TeamSpeak

Some users are bypassing bans by changing IP address and user ID within the program. Is there a way to block VPN/proxy IPs? All of them is pretty much impossible, but most of them is enough.

I know for sure other websites have done this very well. For example hackforums have managed to block almost all VPN and proxy IPs.

My server is hosted on a dedicated Windows Server 2008 R2 machine.
