unable to format sd card

Ok before marking this question duplicate, understand my question:

After formatting my sd card "successfully" from NTFS to Fat32, I put some content on it from my computer. But when I mounted it back to my android device, my old sdcard content was showing as if I didn't do anything.

Problem: SD card does successfully format, no errors at all. But it still acts as if it was not formatted.

More on issue: I used that sdcard in a windows phone, then I performed format on windows computer and after success I moved it on an android phone. Also sd card seems curropted as I Cannot download/install anything in it.

What is the cause of issue and what are my options.
