Apache and uploading files

I have Apache 2.4.20 on Ubuntu 16.04 and I would like to handle two scenarios:

1) a file gets uploaded to Apache by POST request. How can I categorize that file according to MIME content and then, have Apache save that under a custom directory? Note that the file won't get uploaded to https://myserver.com but to something like https://myserver.com/non-existent-folder-name

2) Can you guide me analytically as to how I could setup a form that requests the user to upload a document and when clicking the "upload" button, the file will get saved to a custom directory?
