Assuming you can't directly boot into a linux system, how do you make the bootstrap?

What are the actual steps, assuming you had a set of c header files that described the memory mapped devices in your system, to make the initial ruing keel? I know everyone just boots off a live CD/USB Thumb drive etc. but how did that first bootstrap get made?
